Tayyba Buttar, MD
Psychiatrist
Dr. Tayyba Buttar earned her medical degree from the University of Punjab in Lahore, Pakistan and completed her residency at Berkshire Medical Center in Massachusetts. After completing her degree, Dr. Buttar was involved in a residency program which focused on general psychiatry for an underserved population with a prevalence of trauma, chronic mental illness and substance abuse.
Dr. Buttar has worked with adults, adolescents and the geriatric population. She has practiced psychiatry in both inpatient and outpatient settings and has extensive experience in emergency settings. Dr. Buttar also practices in substance abuse and addiction settings — most notably, she has run a Suboxone outpatient clinic for several years.
From 2006 through 2008, Dr. Buttar was involved in clinical research. In Pakistan, she worked with the Patient Welfare Society where she assisted in providing medications to patients living in poverty, set up health camps in rural and economically deprived areas and conducted social awareness workshops. Dr. Buttar is certified by the American Board of Psychiatry and Neurology, and she’s a member of the American Psychiatric Association.

Keshavpal Reddy, MD
Psychiatrist
Dr. Keshavpal Reddy graduated from Osmania Medical School in Hyderabad, India and completed his residency with Connecticut Valley Hospital, an affiliate of the Yale University Psychiatry Training Program.
Board-certified in both general psychiatry and addiction medicine, Reddy’s expertise includes mood and anxiety disorders, dementia, ADHD, schizophrenia, alcohol and drug treatment, as well as co-occurring disorders. Dr. Reddy is also an expert in adolescent psychiatry with an emphasis on mood and behavior disorders.
He is a member of the American Psychiatric Association, the American Medical Association and the American Society of Addiction Medicine. In addition to his role at Old Vineyard, Dr. Reddy has a private practice in Greensboro and also serves as medical director of psychiatry for Martinsville Memorial Hospital.

Rajakumar Thotakura, MD

After earning his medical degree at Guntur Medical College in Guntur, India, Dr. Rajakumar Thotakura completed an internship in pediatrics at the Bronx-Lebanon Hospital Center. After his internship, Dr. Thotakura finished his residency in general psychiatry at Connecticut Valley Hospital, an affiliate of the Yale University Psychiatry Training Program and also completed his fellowship in child and adolescent psychiatry at St. Vincent’s Hospital and Medical Center in New York.
Dr. Rajakumar Thotakura is a board-certified psychiatrist and neurologist for adults, adolescents and children. He is a member of the American Psychiatric Association and the American Association of Child and Adolescent Psychiatrists. In addition to his role at Old Vineyard, he also has a private practice with Winston Psychiatric Associates in Winston-Salem, North Carolina.

William Yarborough Carlton, MD
Psychiatrist
Born and raised in Winston-Salem, North Carolina, Dr. William Yarborough Carlton received his undergraduate degree from the University of North Carolina at Chapel Hill and his medical degree from Bowman Gray School of Medicine at Wake Forest University. He completed his internship and residency at East Carolina University.
Dr. Carlton specializes in affective disorders, psychopharmacology medication management, anxiety disorders, medical hypnotherapy and long-term dynamic psychotherapy. Board-certified in psychiatry and neurology, he is a member of the Forsyth County Medical Society. Along with his role at Old Vineyard, Dr. Carlton has a private practice with Winston Clinical Associates in Winston-Salem, North Carolina.

Umalakshmi K. Thotakura, MD
Psychiatrist
Dr. Umalakshmi Thotakura received her medical degree from Guntur Medical College in Guntur, India. She completed her residency with Connecticut Valley Hospital, an affiliate of the Yale University Psychiatric Training Program. Following her residency, Dr. Thotakura went on to complete her fellowship in child and adolescent psychiatry at St. Vincent’s Hospital and Medical Center in New York.

Gisela Edith Knebl Kohl, MD
Psychiatrist
A native of Argentina, Dr. Kohl was a chemical engineer before following her passion for medicine. She completed her medical degree at the University of Buenos Aires Medical School in Argentina, and her residency in psychiatry at Jackson Memorial Hospital in Miami, Florida. During her residency, Dr. Kohl trained at the VA, in particular attending the PTSD veteran unit. There, her rotations included psychiatry, pediatrics, internal medicine, family medicine, neurosurgery and endocrinology.
Dr. Kohl specializes in mood disorders, schizophrenia, addiction and dual diagnosis. She is board-certified in addiction medicine and an active member of the American Psychiatric Association.
In addition to her role at Old Vineyard, she started her own practice in 2017. Her vision of psychiatric care involves integrative treatment by looking at the entire patient including lifestyle, nutrition habits and physical activity. Dr. Kohl has helped numerous patients realize better outcomes by treating both the emotional and physical aspects that may contribute to a particular diagnosis.

Tanvir Iqbal, MD
Psychiatrist
Dr. Iqbal graduated medical school with honors from Lugansk State Medical University, Ukraine.
He completed his residency from Delaware Psychiatric Center Newcastle, DE. He completed his Child and Adolescent Psychiatry fellowship from New York Medical College/Westchester Medical institute Valhalla, NY. Board certified in both general psychiatry with child and Adolescent Psychiatry, Dr. Iqbal expertise include ADHD, mood and anxiety disorders, schizophrenia, substance use disorders along with other psychiatric comorbidities.
During his off time, he enjoys traveling, movies and music.
